Covid Vaccine Pfizer for children From 3 to 5 years

From 3 years to 5 years NO prescription is required.

  1. Make sure to download and read the VACCINE INFORMATION FACT SHEET
  2. Bring a copy of the child’s birth certificate.
  3. A parent or legal guardian must accompany the child to be vaccinated.
  4. Fill, sign and bring a copy of the consent form for minors attached
